These formsshould be completed by the Manager/Coach whenever any player is injured during practice or league play. The completed formsshould be forwarded to the Vice President of Administration of the Lincroft Little League.

A medical release form should be completed on every player and maintained by the team manager. The form should be present with the team at every practice and every game.
